At PrimeCare Health, your care team will coordinate the care you and your family receive in our centers. We’ll keep your complete medical history on file so we’ll be able to help you quickly and efficiently every time you visit. We’ll remind you when it’s time for check-ups, tests or treatments and help you set goals to manage long-term conditions. If you need referrals, tests or specialists, we’ll help you get the process started.

Case managers will help assess your health, coordinate care and work closely with home health agencies and community-based agencies to assist you with important services like transportation, housing, utilities, food and managing your daily life.

Your medical home is where you and your family can go for regular checkups or when you’re sick. We provide medical, behavioral health, and oral health care for people of all ages, regardless of ability to pay. Your medical home is also the place to come for referrals, get help from a case manager, or sign up for insurance. For your convenience, we offer appointments during the day, evenings, and on the weekend.  You can also communicate with your provider via the patient portal for easy access.

At your medical home, you have a care team to ensure all your needs are met to the highest quality.  Your care team includes: your primary care provider (your doctor, nurse practitioner or physician assistant), medical assistant, and nurse care manager. Your care team may also include a behavioral health provider and a case manager.

Your team will provide you and your family with care that is evidence-based and follows scientifically proven guidelines. You will be offered routine screening tests and other health prevention services as recommended by the United States Preventative Health Services Task Force (USPSTF), the foremost group in the U.S. for advising medical professionals in providing preventative care.

Primary Care

Think of us as your “family doctor.”  We offer primary care with family physicians and caring trained professionals who address all the medical needs of you and your family. As a team, we’ll be there for you today and into the future. It’s relationship-based care that respects each patient’s needs, culture, values and preferences.

 
 
 
 

HIV PREVENTION PROGRAM

We believe that HIV prevention is part of all primary care services. Our HPP team is committed to increasing awareness and promoting information on HIV risk factors and other sexually transmitted infections. Our HIV prevention services are free for all patients. We can help with full STI and HIV testing, PrEP, and PEP prescriptions to prevent the transmission of HIV, and free condoms. Also, linkage to care and medication assistance to receive medication for free. By promoting a sex-positive culture we intend to recognize sexuality, as a normal and healthy part of the human experience. BEHAVIORAL HEALTH

At all sites, behavioral health consultants work closely with primary care providers to support patients in making lifestyle changes necessary to attain and maintain good health.  Services include programs to effectively manage emotional or behavioral difficulties such as anger, anxiety, depression, stress and bereavement.  There are also support services for behavioral change plans related to smoking cessation, weight loss, alcohol use, exercise and other lifestyle modifications.

 

MEDICATION-ASSISTED TREATMENT (MAT)

At our West Town and Belmont Cragin sites, we provide support through individual and group treatment through recovery-based services. Both substance use disorder treatment and medication-assisted treatment are offered, including treatment for opioid use disorder using approved medications. DENTAL CARE

Oral health is an important part of overall health. At PrimeCare Health, we offer basic preventive, restorative and emergency dental services to patients of all ages. This includes exams, cleanings, fluoride application, fillings, sealants and extractions. We currently accept all pediatric patients, 0-18 years old, regardless of where they receive primary care services, and adults (18+) who are also receiving primary care services from PrimeCare Health.

PRENATAL CARE

Prenatal care is an important part of staying healthy during pregnancy. Our doctors and nurses will monitor your baby’s development and do routine testing to help find and prevent possible problems. These regular checkups are also a great time to learn how to ease any discomfort you may be having and ask questions about your pregnancy and the birth of your baby.

 
 
 
 

INSURANCE/
AFFORDABILITY

We believe that everyone deserves access to high quality health care. At PrimeCare Health, no one will be denied services because of their insurance status or inability to pay. If you have insurance, we can help you understand it and use it. For patients who are uninsured or underinsured, we offer a sliding scale fee based on your income and family size. If you are unsure if you qualify for the sliding fee discount scale, our Access and Enrollment Specialists can help. We can also answer your questions about Medicare and Medicaid. BILLING

At PrimeCare Health, we want to make sure that our patients are always in the loop about any payments they need to make.
