Quality Metrics

For 30 years, PrimeCare Health has provided quality and affordable care to underserved communities in Chicago. Our multilingual providers and staff address unmet need by serving patients of all ages and helping them access our services. In 2022, we were named a Health Center Quality Leader by the Health Resources & Services Administration (HRSA). We were given this recognition for our high rates in clinical quality measures.

The previous year was our first time being recognized as a Health Center Quality Leader. We also received Community Health Center Quality Recognition (CQHR) badges in Target Blood Pressure, Statin Therapy, Target Type 2 Diabetes, Advancing HIT for Quality, and PCMH Recognition.

Although these awards are a great honor for our organization, we thank our dedicated staff and trusted patients who work together every day to foster positive health outcomes for more than 20,000 people across the northwest side of Chicago.

2025 National Quality Leader in Heart Health.

In 2025, PrimeCare Health was recognized as a National Quality Leader for checking on and ensuring heart health in our community members.

Health Center Quality Awardee in 2025.

In 2022, PrimeCare received a Gold Medal as a Health Center Quality Leader. The Gold Medal is awarded to the top 10% of health centers in the country. We've continued to receive this distinction every year!

2025 Check Change Control Cholesterol Gold Recognition

PrimeCare Health received Check. Change. Control. CHOLESTEROL™️ GOLD recognition for improving quality of care through awareness, detection, and management of high cholesterol by education and empowering patients with evidence-based strategies and tools.

2025 National Quality Leader in Diabetes Health.

In 2025, PrimeCare Health was recognized as a National Quality Leader for helping community members manage their type 1 & 2 diabetes.

PCMH Recognition

PrimeCare has received Patient-Centered Medal Home (PCMH) Recognition, meaning our organization met standards in these six areas: Team Based Care and Practice Organization, Care Management and Support, Know and Manage Patients, Care Coordination and Care Transitions, Patient-Centered Access and Continuity, and Performance Measurement and Quality Improvement.

2025 Target: Type 2 Diabetes Participant

PrimeCare Health received Target: Type 2 Diabetes Participant status from the American Heart Association for improving quality of care for patients with type 2 diabetes and cardiovascular risk factors.

2025 Awardee in Preventive Health.

In 2025, PrimeCare Health was recognized as a leader in preventive health for meeting or exceeding in the following quality measures: dental sealants (6-9 years); weight assessment and counseling for children; early childhood development screenings (<18 years); depression screening and follow-up; tobacco screening and cessation; an adult BMI screening and follow-up. Achieving this quality award shows that our patients can entrust us with their wellness today and moving forward.

2025 Target Blood Pressure Gold Recognition

PrimeCare Health received Target: BP GOLD+ recognition from the American Medical Association and American Heart Association for achieving 70% or greater blood pressure control and committing to accurate measurement among adult patients.

2024 Certification of Compliance for an Operational Site Visit

PrimeCare had an Operational Site Visit (OSV) in 2024, where it was determined that our organization demonstrated full compliance with all of HRSA’s Health Center Program requirements. This shows a dedication to excellence, operational integrity, and continued service to our patients and community.

We had 31,984 patients in 2024.
We had 105,292 visits to our centers in 2024.
In 2024, 81% of our patients had public insurance or no insurance.

Patients aged 12+ with a visit in 2024 who were screened for depression and, if positive, have a follow-up plan documented.

92% WEIGHT ASSESSMENT KIDS

Patients aged 3-17 with a visit in 2024 and a documented BMI percentile, nutrition, and exercise counseling.

81% USE OF ANTIPLATELETS IN PATIENTS WITH IVD.

Patients with Ischemic Vascular Disease (IVD) with a visit in 2024 who were prescribed aspirin or other antiplatelets.

Patients aged 13+ with a visit in 2024 who were screened for tobacco use and, if positive, have a follow-up plan documented.

89% statin therapy to treat cardiovascular disease.

Patients aged 21+ at high risk of cardiovascular events who were prescribed a statin in 2024.

75% CERVICAL CANCER SCREENING.

Patients with a cervix aged 21-29, with a visit in 2024, who had PAP SMEAR within the last three years, or aged 30-64 with PAP SMEAR & HPV co-testing within the past 5 years.

63% breast cancer screening.

Women aged 51-73 with a visit in 2024 who had a mammogram to screen for breast cancer.

73% CONTROLLING HIGH BLOOD PRESSURE

Patients with hypertension with a visit in 2024 whose blood pressure was regulated to <140/90 mmHg.

87% BMI SCREENING & FOLLOW-UP.

Patients with a visit in 2024 whose BMI was measured and, if overweight, had a follow-up plan documented.
