Quality Metrics

For 30 years, PrimeCare Health has provided quality and affordable care to underserved communities in Chicago. Our multilingual providers and staff address unmet need by serving patients of all ages and helping them access our services. In 2022, we were named a Health Center Quality Leader by the Health Resources & Services Administration (HRSA). We were given this recognition for our high rates in clinical quality measures.

The previous year was our first time being recognized as a Health Center Quality Leader. We also received Community Health Center Quality Recognition (CQHR) badges in Target Blood Pressure, Statin Therapy, Target Type 2 Diabetes, Advancing HIT for Quality, and PCMH Recognition.

Although these awards are a great honor for our organization, we thank our dedicated staff and trusted patients who work together every day to foster positive health outcomes for more than 20,000 people across the northwest side of Chicago.

 
We served 27,915 patients in 2022. We are committed to the family medicine approach, which is based on a continuous, comprehensive, and holistic patient-provider relationship.
We had 111,969 visits to our centers in 2022. We seek to improve the health and wellbeing of all individuals & communities that we serve in partnership with aligned health and social service providers.
85% of our patients have public insurance or no insurance. We work with our patients to provide quality, affordable care to a medically underserved population.

Patients aged 12+ with a visit in 2022 who were screened for depression and, if positive, have a follow-up plan documented.

Patients aged 3-17 with a visit in 2022 and a documented BMI percentile, nutrition, and exercise counseling.

Patients aged 15-65 with a visit in 2022 who were screened for HIV.

Patients aged 13+ with a visit in 2022 who were screened for tobacco use and, if positive, have a follow-up plan documented.

87% statin therapy to treat cardiovascular disease.

Patients aged 21+ at high risk of cardiovascular events who were prescribed a statin during the year.

Patients with a visit in 2022 whose first prenatal visit was in their first trimester.
