PrimeCare Wicker Park is located on the second floor in the Medical Office Building of Nazareth Family Center, located on Oakley Boulevard between Haddon Street and Thomas Street.

By Bus: #49 Bus (Western Avenue) to Western at West Haddon Street

By Train: Blue Line to Damen or to Western

Free parking is available for patients. The parking lot is located east of the clinic on W Thomas St, approximately 2258 W Thomas St, Chicago, IL. If the gate is closed, please call 312-770-2457, and they will open it for you. Street parking is also available, but please pay attention to permit signs.
