PrimeCare is a community health center serving Chicago. We provide primary care for individuals and families across the lifespan—from infants and children to adults and seniors.
Our care team includes doctors, nurse practitioners, physician assistants, and other health professionals who work together. We focus on keeping you well, treating illness, and helping you manage chronic conditions.
PrimeCare welcomes everyone, no matter your income or insurance status. We accept most insurance plans, including Medicaid and Medicare. If you do not have insurance, we can help you apply for coverage or use our sliding fee scale.
