Notice of Dental Closure

PrimeCare Dental Patients,

We hope this letter finds you well. We are writing to inform you of an upcoming change to dental services at PrimeCare Community Health. Effective June 1, 2026, our West Town Dental location at 1431 North Western Avenue, Suite 401, will close. Dental services will continue without interruption at our Belmont Cragin location at 5635 W. Belmont.

You may continue to receive the same high-quality dental care at our Belmont Cragin location. The providers currently serving patients at our West Town site will be transitioning to the Belmont Cragin location to ensure continuity of care. We sincerely hope you will continue your dental care with us at Belmont Cragin, as maintaining regular dental visits is important to your overall health.

Additionally, we are pleased to share that a private company, Sonrisa Dental, will be moving into Suite 401 at the West Town location. Although we are separate entities, PrimeCare and Sonrisa Dental, will partner so our patients will have expanded access to more advanced dental services, including extractions, extensive pediatric dentistry with sedation, crowns, dentures, partials, implants, and select root canal treatments.
